Woodside Bible Fellowship collects and retains the personal information provided on this registration form about the child/youth participant and their parent or legal guardian to register the participant, communicate about program details, administer and support the program, maintain appropriate records, and help protect the safety and well-being of participants, volunteers, and staff. Information collected may include the participant�s name, age or date of birth, parent/guardian contact information, registration details, and attendance information. Information will be used only by authorized personnel for these purposes and will not be disclosed except with consent, where necessary to administer the program or address a safety concern, or where required or permitted by law.
